What can I do if payment for my claim was denied by MMM of Florida?

If MMM of Florida denied your payment claim, you have the right to request a claim adjustment within 365 days from the date of service. In addition, you have the right to appeal or dispute payments, according to the corresponding situation. We are providing our mailing address, fax number and phone number for any submission of information or additional questions you may have.
